Prescription Information – for residents of the U.S. and its territories


U.S. federal law restricts Avazzia medical devices to sale by or on the order of a physician or licensed healthcare provider. This means that a prescription is required to purchase Avazzia medical devices.

In addition to a prescription, most insurance companies require additional medical records to evaluate and determine insurance coverage. For example, they will want to know thinks like date of onset, diagnosis information, other treatments tried before, the last time the doctor saw you and whether the doctor feels that the device would be medically necessary.

This prescription form is useful for including medical history information that your insurance company may request:

Under some circumstances, medical devices may be covered by insurance.

Avazzia does not file insurance claims.

(For those living outside the U.S. or its territories, a prescription is not required.

For private insurance (other than Medicare):

Some insurance policies may provide benefits towards the cost of a BEST-PRO 1. Check with your agent or the human resource office at your company.

Achieve Chiropractic helps patients file paperwork with their insurance company in order to receive approval/reimbursement for purchase of an Avazzia BEST-PRO 1. You will need to gather information for submitting to insurance:

  1. A prescription filled out, dated and signed by the doctor,
  2. Patient copayment.
  3. A copy of the front and back of insurance card.
  4. A copy of identification.
  5. A completed Assignment of Benefits for filing with your insurance company. This form is available from Leanne at Achieve Chiropractic.

FAQ. What if I don’t live close to Garland, Texas?  

If you have a prescription from your doctor, you may still contact Achieve Chiropractic to help you file for insurance.

FAQ. Is Achieve “In-Network”?  

Achieve is In-Network for a number of insurers. After a review of your information, they can work with you for the best way to proceed.


Even though Avazzia does not file insurance, and Medicare does not cover the cost of the devices, Avazzia values you and your business and wants to help you.

If you are 65 years or older, Avazzia wants to thank you for you’re your years of service with a senior appreciation discount for those who qualify.
